Hereby, I agree to purchase a year-membership for M.S.V.V. Fyrfad for the amount that is determined at the General Members Meeting, which is €85,00, in addition to an amount of €40,00 for the set Voluntary Contribution, for a hafl-year membership this amounts to €60, in addition to an amount of €40,00 for the set Voluntary Contribution, before the fixed deadline of the 1st of October, in order to complete my subscription for M.S.V.V. Fyrfad.

Hereby, I authorize M.S.V.V. Fyrfad to send recurrent collection instructions to my bank to debit my

account on a recurrent basis in accordance with the instructions of M.S.V.V. Fyrfad:

1. To debit my account with Fyrfad subscription for the amount that is determined at the

General Members Meeting, which is €85,00 (+40,00 €), assuming the payment via the website failed or will not

be offered in the upcoming academic year 2021-2021.

2. To debit my account with a fine of €45,00 if I am not present to be the arbiter/counter of the game for

which I was responsible, and I did not arrange a substitute.

4. To debit my account with the amount of the fine Fyrfad gets from the MUSST if I do not have a

valid sports card before the 1st of October.

5. To debit my account with a part of the costs which are the consequence of loss of or damage to

my team’s shirts, shirt-bags, player’s cards or any other materials that belong to M.S.V.V. Fyrfad.

I am informed that, if I conclude my membership in writing one month before the beginning of the new volleyball season (The 1st of August) this authorization will be ended. In order to unsubscribe, a formal termination of membership e-mail must be sent to secretaris@fyrfad.nl. This authorization will only be reinforced after I am given a place in a team. No money will be debited while I am still on the waiting list. The 40 Euros that are paid for the voluntary contribution in addition to the regular member fees will be refunded at the end of the academic year if I meet the necessary requirements (M.S.V.V. will offer you opportunities to do so).

As part of your rights, you are entitled to a refund from your bank under the terms and conditions of your agreement with your bank. A refund must be claimed within 8 weeks starting from the date on which your account was debited. Ask your bank for the conditions. If you paid your contribution via the website, then you can contact penningmeester@fyrfad.nl to receive this refund.

M.S.V.V. Fyrfad keeps this declaration in her members administration

 In order to unsubscribe, a formal termination of membership email must be sent to secretaris@fyrfad.nl within three weeks of signing.
